Gym Management SaaS — QA, Security Audit & Production Readiness

Comprehensive audit executed on 2026-10-15 • Laravel 13.31.0 • PHP 8.5

Executive Summary

Overall posture and production readiness recommendation

Overall Status

PASSED

All automated tests pass. Multi-tenant isolation verified.

Test Results

447 passed / 0 failed

1483 assertions across Unit and Feature suites (+10 new regression tests).

Security Posture

GOOD

One critical cross-tenant issue identified and remediated. No critical/high issues remain.

Production Readiness

APPROVED WITH CONDITIONS

Safe to deploy. Address documented MEDIUM items post-release or before depending on risk tolerance.

Test Execution Results

Full suite verification

447 tests, 447 passed, 1483 assertions (duration ~44s)

New Regression Tests (tests/Feature/SecurityAuditTest.php)

10 tests, 69 assertions covering cross-tenant IDOR (read/mutate across 18 resource types), branch switch protections, gym_ids cross-client filtering, role boundaries, unauthenticated redirects, and mass-assignment sanity.

Findings & Remediation

Issues discovered, severity, and fixes applied

CRITICAL — Cross-tenant staff assignment

FIXED

Root cause: UserController::store/update did not restrict gym_ids to caller's client; foreign gyms could be attached via gym_user pivot.

Impact: Boundary violation enabling potential cross-tenant access path (policies still blocked most reads by client_id mismatch).

Fix: Added onlyOwnGymIds() filter to intersect submitted gym IDs with caller's active client gyms in store/update. Fixed User::assignedGymIds() return type.

Evidence: SecurityAuditTest regression passes; full suite green.

MEDIUM — Attendance check-in race (TOCTOU)

DOCUMENTED

Location: AttendanceService::processForMember/processForStaff

Issue: Open session check followed by create without row-level locking; concurrent requests could create duplicate open sessions.

Recommendation: Add lockForUpdate() or enforce DB-level uniqueness (partial unique index on (gym_id, member_id) where check_out_at IS NULL). Hard to validate on SQLite.

MEDIUM — Trash force-delete permissions (design)

DOCUMENTED

Observation: TrashRegistry allows non-admin roles (receptionist/accountant) to force-delete certain modules. Scoped to active gym and role-gated; no bypass found. Recommend policy review for alignment with business requirements.

Deliverables

Reports generated in repository root

  • QA_REPORT.md — Overall status, tests executed, findings, fixes, coverage gaps, production readiness.
  • SECURITY_AUDIT.md — AuthN/AuthZ, tenant isolation, input validation, CSRF, uploads, residual risks, remediation evidence.
  • TEST_PLAN.md — Feature inventory, role matrix, risk-based test matrix, new regression tests, execution commands.
  • reports.html — This consolidated HTML report.
  • tests/Feature/SecurityAuditTest.php — 10 automated regression tests (committed).

Files Modified (Fixes Applied)

FileChange
app/Http/Controllers/Client/UserController.phpAdded onlyOwnGymIds() helper; filter submitted gym_ids to caller's client (store/update).
app/Models/User.phpFixed assignedGymIds() return type to Illuminate\Support\Collection.

Recommendations (Non-Blocking)

  • Concurrency hardening: Add row-level locking or partial unique index for open attendance sessions (MySQL).
  • Dependency scanning: Run composer audit in CI to catch known CVEs.
  • Policy review: Confirm trash force-delete permissions align with business requirements.
  • Extended hardening: Consider additional upload MIME validation and PDF/report fuzzing for completeness.
